Enhancing neurosurgical precision with human-centered design

Spontaneous intracerebral hemorrhage (ICH) is a life-threatening emergency with a 30-day mortality rate of nearly 40%. Fewer than 40% of survivors regain long-term functional independence. Timely intervention within the first 24 hours is crucial to improving outcomes, yet traditional neurosurgical procedures require complex, costly equipment and extensive surgical expertise. A minimally invasive, integrated solution is needed to enable faster, more accessible treatment.

Transforming neurosurgery with an integrated, user-centered approach

Pilotfish collaborated on the development of a disposable use endoscopy tool that serves as the core of the Neuroblade System - an all-in-one platform for minimally invasive neurosurgery. The device was designed to simplify critical interventions such as ICH evacuation by integrating multiple surgical capabilities into a single-use endoscopic instrument. Our human-centered design process focused on: • User Research: Conducting in-depth usability testing with neurosurgeons to refine ergonomics and functionality. • Proof of Concept: Developing and evaluating multiple design iterations through sketches and prototypes. • UX & Product Design: Ensuring a stable and comfortable grip for precision control in high-stakes surgical environments.

Streamlining neurosurgical procedures for faster, more accessible treatment

The disposable neuroendoscope is designed for use with the Neuroblade System’s portable 15" high-definition touchscreen monitor (Neuropad), which allows surgeons to interact with real-time visuals, capture intraoperative screenshots, and control the integrated functions. This multifunctional endoscope development for ICH allows surgeons to perform a variety of functions like bipolar coagulation, morcellator debrider, lens cleaning, and suction/irrigation, all within a single-use instrument. This prominent level of integration enables single-surgeon operations and cuts the need for multiple devices, making the system especially valuable in regional hospitals. Several ergonomic design concepts were assessed with direct input from neurosurgeons during the disposable endoscopic tool development process, ensuring optimal usability and performance during prolonged procedures. "One Doctor, One Hour" was the guiding principle behind the Neuroblade System, enabling critical neurosurgical interventions to be performed quickly and independently. Faster procedures and fewer personnel requirements increase treatment accessibility and have the potential to significantly improve patient survival and recovery.
